Origin-evidence checklist

Build a dated record for one claim and one material.

• Write the exact claim, including any qualifying words.
• Identify the claimed step: manufacture, testing or dispatch.
• Record the named entity and location for that step.
• Attach the original source URL and observation date.
• Identify the product and lot to which the source applies.
• Record what the source does not establish.

Leave uncertainty visible

If a record only identifies a laboratory, keep the production field unresolved. Do not fill it from a guess about the supplier’s brand or office location.
